Registration Form

The Registration Form (PDF) is required for all overnight trip, otherwise complete your registrations on-line or in our office and this form is not needed.  All fields should be completed, and the release must be signed. Registrations will not be accepted if they are not accompanied by payment in full. Register on-line at www.scpr.info.

Parks & Recreation Facility Use Request

The Parks and Recreation Facility Use Form (PDF) is required if you wish to reserve a Parks and Recreation facility (ex: Charterhouse gymnasium or a shelter or field at the County Park) and must be accompanied by payment in full. Rates are listed on the form.

Public School Rental Request Form

The Public School Rental Request Form (PDF) is required if you wish to reserve a facility owned by the School Board. Once complete, this form will be processed, and you will be notified if your request has been accepted or denied. Third-party requests will be handled on a "first-come, first-serve" basis. School and Parks and Recreation activities take precedence.

Membership Change Request Form

The Membership Change Request Form (PDF) is required for all changes made to your account relating to memberships. Form must be submitted 10 days prior to bulling cycle renewal date. Return form to our office.

Refund Request Form

The Refund Request Form (PDF) is required for all cancellations and refunds requested. Be sure to read the refund policies at the top of the form before submitting your request; if additional paperwork is required, please be sure to attach it to the refund request form.

Punky Riley Scholarship Form

The "Punky" Riley Scholarship Program Form (PDF) was established to ensure that all County youth would be able to participate in Shenandoah County Parks and Recreation's programs and events, and the program was recently updated by Shenandoah County Parks and Recreation to allow more people to participate in the scholarship process. The Fund was established in the name of the late Wilbur O. "Punky" Riley, who was a huge supporter of youth parks and recreation programs.

Brochure Advertising Form

Shenandoah County Parks and Recreation produces their Recreational Guide three times per year which is distributed via direct mail to over 18,000 households in Shenandoah County. The department has an advertising program in place if you would like to advertise in our publication.
